Each person must fill out the Registration & Prize Form AND sign it to be eligible for the tournament.

REGISTRATION FORM & LIABILITY RELEASE

(Please read carefully)

I acknowledge that I am voluntarily participating in the Carpenters North Star Walleye Classic. I am aware that participation creates the risk of personal injury or damage to my property. I assume all risk of personal injury or damage to my property that may result from my participation. I hereby release and  discharge the North Central States Regional Council of Carpenters, its officers and employees, the  tournament director, the Carpenters North Star Walleye Classic and all tournament sponsors, as well as Trapper’s Landing, from any liabilities, injuries, damage or other loss sustained by me caused by the negligence of the released parties.

* If you want a registration confirmation and a copy of the rules, you must provide a legible email address.

Incomplete Tournament Entry Forms will be returned and not accepted. The prize winners are responsible for all taxes and licensing, and will be given a Form 1099.

PAYMENT

Entries: $150/person or $300/team
Payment must be received to confirm registration.
